Memory Care at Cornerstone
Cornerstone’s Memory Care offers a thoughtfully designed, entrance-controlled environment for individuals living with Alzheimer’s disease or other forms of dementia. We strive to create a calm, welcoming setting that is easy to navigate. Familiar, simplified spaces promote comfort and well-being. Each resident enjoys a private apartment that can be personalized with meaningful items to make it home.
Our specially trained memory care team takes time to learn each resident’s unique life story, building meaningful connections and honoring who they are. Daily routines include individualized, purpose-driven activities that help residents stay engaged, encourage the use of retained abilities and foster a sense of accomplishment.

The monthly apartment rental fee includes the following:
- Private studio apartments specifically designed for those requiring memory care
- Private bathroom with walk-in shower
- A structured daily plan offering meaningful, stimulating activities designed to meet each resident’s abilities
- Secure environment with multiple community spaces designed with the resident in mind
- Daily snacks and beverages available
- Secure outdoor patio
- Visual cues to orient residents to their surroundings
- All utilities and landline phone service
- Basic Wi-Fi throughout community
- Basic satellite TV service, including 40 channels
- Assistance with transportation scheduling as needed
Health services package
- Initial health care assessment with individualized care plan
- Personal care staff on duty 24 hours per day
- AM/PM dressing and grooming up to 30 minutes/day
- Shower assistance up to twice weekly
- Bathroom Assistance III
- Medication Administration up to twice daily (Basic, Preferred Pharmacy: up to ten medications)
- Escorts within memory care neighborhood
- Reassurance Checks (Maximum every two hours)
- Monthly vital sign management, reviewed by nursing staff
- Weekly housekeeping services with daily trash removal
- Three loads of laundry weekly (two personal and one linen)
- Weekly linen change
- Daily Bed-making
